ATTENTION. PLEASE CAREFULLY READ before booking or requesting a free online consultation: AGE REQUIREMENT:
•Over 18 years old.
IF YOU HAVE ANY PREVIOUS WORK DONE ON YOUR EYEBROWS (NOT VIRGIN BROWS):

Send 3 pictures of your full face without make-up or filters. One picture should be from the front of your face, one on your left side, and one of your right side. Pictures should be sent from the “Free Consultation” tab, but they can be sent directly by email/text message.

If you have any of the following, you may not be a good candidate for this treatment as it may not be safe or may not last long:



DISQUALIFICATION LIST:


• Anyone allergic to pigmentation and makeup.


• Anyone with glaucoma and/or taking blood-thinning medications.


• Anyone with skin diseases such as eczema or psoriasis on the area to be treated.
• Anyone with active skin cancer in the area to be treated.


• Anyone with undiagnosed rashes or blisters on the area to be treated.


• Anyone who has experienced post-inflammatory hyperpigmentation.


• Anyone with transmittable blood conditions. (i.e. Hepatitis or HIV).


• Anyone diabetic.


• Anyone with mitral valve disorder or high blood pressure.


• Anyone who is a hemophiliac.


• Anyone with healing disorders.


• Anyone prone to keloids.


• Anyone taking skin medications such as antasure, ro-accurate, and steroids, which thin and make the skin very sensitive.


• Anyone currently under or scheduled for radiotherapy or chemotherapy treatment.


• Any radiotherapy or chemotherapy must be no less than 42 days from the date of the treatment.


• Anyone epileptic or who has experienced faint spell or seizures.


• Anyone pregnant and/or lactating.


• Please notify us if you have/had allergic reactions to topical anesthetics.


If none of this applies to you and you wish to book please do so!

24 HOURS BEFORE YOUR APPOINTMENT:

• DO NOT CONSUME any of the following: Coffee, Cafein, Energy Drinks, Alcohol, Medication, Aspirin, Niacin, Tylenol, omega3 (sh oil), or Vitamins. (Nothing else but Water)
• Failure to do so will result in increased bleeding and the anesthetic will not be as effective. No blood-thinning medications or painkillers. All daily medications should be reported to avoid unexpected surprises.

• Do not tan or have intense sun exposure 5 days before treatment. This may cause scarring and will affect pigment selection and retention.
• No waxing 15 days before treatment. We will shape and clean your eyebrows at the appointment.
• No chemical peels, dermabrasion, laser or any other intense treatments 2 weeks before treatment.

• No Botox 3 weeks prior.
• Hower and x your hair as you need to take extra care not to get your brows wet for several days.
• Any blemishes, pimples, medical or skin condition in the brow region will be sent home. We cannot tattoo over that and risk you getting an infection.
• I will be taking before and after photos for my portfolio so please wear your makeup if you’d like. I will clean your brow region before I start the mapping procedure.
• Bring in photos and be prepared to tell me the kind of brows you would like to have so I can ensure I map your brows the best to your liking. As an artist I will inform you of what brow shape better suits your face but just know that at the end of the day I am here to service YOU!

DEPOSIT/PAYMENT:

• $60 a non-refundable deposit is required to secure your spot. The remaining balance will be paid on the appointment day, we take CASH ONLY.

• Appointments must be canceled  24 hours after booking. Full Price service will be charge if NO-SHOW, CANCEL-LAST MINUTE 

CANCELLING OR RESCHEDULING YOUR APPOINTMENT:

• You will have 1 chance to reschedule your appointment a minimum of 72 hours before your appointment. If you try to reschedule less than 72 hours before your appointment, you will lose your deposit. Less than 48 hours, you will be charge full price.

• You will not be allowed to reschedule a second time.

NO-SHOW

• If you do a "No-Show" your appointment will be cancelled and your deposit would be forfeited, full price will be charge. If you wish to reschedule after a missed appointment please note that the service must be paid IN FULL before booking again. Failure to do so will result in me cancelling your appointment and you would not be able to book any future services with me.

ARRIVAL:

• No guests, kids, or pets are allowed in our suite to prevent any cross-contaminations or unwanted distractions.

• You must arrive on time. If you're LATE MORE THAN 15 MINUTES to your appointment, you will lose your deposit and your appointment will be canceled.

• Our suite is located inside Salons by JC - Chicago which is located on the second floor. From the street level, there will be an elevator that will take you up. There will be someone at the front desk that will escort you to our suite, just mention you have an appointment at Suite # 42 GRACE LASH BROW

PARKING:

• There is street parking nearby. There is also The BROADWAY Public Parking (2848 N Broadway), which Salons by JC shares. If you decide to use the parking, we can validate your ticket so that your cost is $5 during the week and $9 during the weekend. Keep in mind these rates are for a stay of no more than 3 hours (which should be more than enough as long as you arrive on time). If your stay is beyond the 3-hour limit other charges will apply, which are beyond our control.

DISCLAIMER:

• During the first session, the procedure does not start until you have agreed on the shape that is right for your facial features. We will design the shape based on your bone structure and your desired brows shape. Please make sure you are 100% satisfied with the shape and also with the technique you have to choose before the procedure begins. We will be asking you for your approval once the shape design is completed. After the procedure, you will have one last chance to make sure you like the results, and you are allowed to ask for the last minor adjustments. Once we have carried the services and you have left the appointment there is nothing we can do to x your eyebrows until you come back for another session.

• Everyone is different and can respond to topical numbing and tattoo pigment ink differently. How much you bleed will affect the outcome result and therefore require additional appointments to achieve the desired result.

• A second session is called “First Touch-Up” which is necessary between 45 weeks after your first session to ensure the desired result. The cost of the second session will be determined on the timeframe of your return. Please review our website for more details. It is your responsibility to book this with enough notice within the timeframe.

• The second session “First Touch-Up” is not to change the shape or color of the work done during your first session. If you would like to change the shape of your enhancement during your Touch-Up appointment, you will be charged accordingly as an extra service.

• Our client’s satisfaction is our priority, but all results will vary per person depending on their skin characteristics (dryness, oiliness, thickness, sun-damaged, color, and chemical composition) natural skin undertones mixing with pigment color, personal PH balance of the skin, fruit acids, AHAs/Retin-A use, alcohol intake, smoking, sun exposure, improper skin care, blood type, lifestyle, existing hair, bone structure, face shape, any medications are taken, the number of times the area had been worked on, and follow up of the aftercare instructions. We cannot guarantee a 100% successful result on all of our clients because everyone’s skin takes in Permanent Make-Up directly. Also, there are no refunds as we have carried out any services.

* Due to Covid-19, Clients must wear a clean mask to their appointment and will be asked to sanitize their hands immediately when entering the salon.
* If you are sick, coughing, difficulty breathing, having a fever, taking care of someone who is sick, quarantined due to the Covid-19, or in a household with someone who has symptoms, please reschedule your appointment immediately.
